The Graduate membership form is for Irish physiotherapy graduates who graduated from University College Dublin, Trinity College Dublin, University of Limerick or Royal College of Surgeons in Ireland. If you require the 2012/2013 Graduate form for membership for the month of June 2013 only please email info@iscp.ie.

This application form is not relevant to Eligible Applicants or Renewal of membership.

PLEASE NOTE UNDERGRADUATE MEMBERS FOR 2011-2012 DO NOT HAVE TO PAY THE REGISTRATION FEE ON THE IRISH GRADAUTE FORM 2012/2013. 

If you graduated before 2005 please provide a certified copy of your degree/qualification with the forms.
