Post 1 -Initial Base - Páiste team primary care service, Clondalkin, Dublin 22, Community Healthcare Network 5.

Post 2 - Initial Base - Páiste team primary care service, Kilnamanagh/Tymon Primary Care Centre, Tallaght, 

The Páiste team. This post will be part of a new multi-disciplinary Primary care Paediatric team supporting existing primary care services for children who have a range of social and therapeutic needs. Synchronised intake and intervention from the team provides a 360 view of the child across home, school and clinic settings. Candidates who are flexible, innovative, and interested in cohesive team working may be interested in this post. Other team members include Occupational Therapy, Psychology, SW, SLT and Dietetics. Effective use of team skills and resources will be central to the team ethos and the collaborative goal setting supports the child and family with co-ordinated care closer to home.
